• up to 3 passports for 1 ticket
• 2 photos
• Tips on filling out visa application (from the agent)
  
  -. Write your company name as well as your profession. (NO journalist or photographer)
  -. Write any hotel name (e.g. Traders Hotel) for Myanmar address
  -. Write Male or Female for sex, not M or F
  -. Complexion: light, fair, or dark
  (The visa clerk also asked me to write the hotel I was staying in Bangkok, but this seems to be just for an emergency contact.)
